Health and Safety JHSC Representative Luncheon

Unifor Local 5555 is hosting a luncheon for JHSC representatives on October 17, 2025. We have secured seating and lunch for 30 folks at TwelveEighty Bar and Grill on campus. The Union is offering release time for representatives to attend a 2 hour session from 12-2pm. Please indicate the time and length of your usual lunch hour so that we can arrange release for the balance of the time. Also, please clear it with your supervisor prior to completing the form. If you aren’t able to make it to this luncheon, there will be more opportunities in 2026. We have a short timeline for RSVPs, please complete this form by October 9 at 12pm.

Any incident that causes injury, could have caused injury or workplace hazard should be documented with an incident report.

If you are prescribed a medication not covered under the RX05 formulary used by Sunlife, you may be able to have it covered if there is a medically supported reason.  This is the appeal form to use.
